I live in a Community Association, how do I obtain proof of insurance for my Mortgage company?

In order to complete a certificate of insurance for your mortgage company, we need to receive information specific to your unit and mortgagee's request.  There are 3 ways to submit this information to our office: 

• Complete our online Certificate request form 
• Fax or E-mail a request with the required information
• Call our office at 845-265-2220 and dial ext 126
